Submit a Birthday Announcement


Guidelines:
1. There is an $11 fee for all birthday announcements for children ages 1-10.
2. If parents have different last names and both are listed on the form, BOTHmust sign permission to publish the information. If a grandparent or other relative is submitting the form, he or she may obtain parents signatures and witness they are legal.
